Day 100 - Spit ups & Parent Training

Friday, April 9th 2021

Today our Primary is back after a long stint of random nurses! James has thrown up/spit up at least 5 times. Sometimes it's just breastmilk and other times it's blue because of the morphine. 

After speaking with the Doctor we increased the time the feed will run, so the 56ml bolus feeds will run over 45 minutes instead of 30 minutes. That has helped James so far!

James is ROCKING in the NICU like a Boss.

Ventilator Settings:

FiO2: 32

PEEP: 10

Rate: 40

Tidal Volume: 30

Latest 4am CO2 Blood gas: 

Yesterday it went up to 77.1 from 68.9 and this morning 74.9.

Besides the throwing up, James has the shakes, little tremors in his arms.

Pain Meds before moving to PICU:

Clonidine 2/21/2021 5 mcg q 4 hr

Morphine Sulfate 1/2/2021 0.12 mg q 3 hrs

Discharge from PICU:

clonidine 20 mcg/ml susp, 1 ml 6 mcg 0.3 mL, GTUBE, q6hr (interval)

morphine 50 mg [0.05 mg/kg/hr] + syringe, empty sterile 50 mL 1 EA + D5W 45 mL 45 mL, IV, 0.16 mL/hr

PRN: (11)

Current Dose:

Clonidine 2/21/2021 46 6 mcg q 6 hr

Morphine Sulfate 1/2/2021 96 1.4 mg q 3 hrs

He will be weaned off these starting soon. Right now we are trying to get him stable with the vent settings.

Today James sat in his new boppy for Physical Therapy (Thanks Keri!). Some things they said we can work on with him is light cycling (his days and nights are a little backwards), having planned activities during the day, keeping his head flexed forward because he keeps arching his back during his broncos spasms. 

Mom has been getting trained on suctioning his trach and mouth, and giving medications through the g-tube.

The past two days James has lost weight probably due to the diuretic. We are now back to 3350 grams (7lb 6oz).

On a creepy note mom's phone takes pictures when she says the word "James". And she did not set this up!
