Day 618 - Vampire Baby 🦇 Not Feeling Well 🤒

At around 6pm we contemplated going to the ER due to his respiratory distress. 🚑 It seems to always be on a Friday evening once our pediatrician has closed and we are reliant on the ER (and night shift, on top of it 🙄). James ended up having a fever of 101, high respiratory rate, high heart rate, increase in secretions, needed an increase in oxygen, and was having lung retractions. The increase in secretions and increase in oxygen started on Wednesday when Mom was up with him the entire night. We really tried to think about what they would do at the ER for him. Because of the lung retractions they would probably do an x-ray, but it wouldn't show anything because we knew this was a trach infection. They would do a sputum culture and it would come back that he was colonized with Klebsiella and probably another bacteria. Would an antibiotic work? Well he has already been on Cipro twice this year (April and August) and even though it may help for a little bit it will create antibiotic resistance. Also gram-negative bacteria strains are already resistant to a lot of antibiotics.
He seemed a little fussy but after giving him Tylenol for his fever he was fine. We watched him closely for a few hours until he fell asleep and his numbers got back to normal. At that point we decided we would just monitor him closely at home and see if he could stay stable until Monday to call the pediatrician. James is definitely fighting another trach infection (which is common).
We need to get a sputum culture to find out which strain of bacteria he has, so we know which antibiotic will kill it. He is colonized with Klebsiella which is a gram rod negative bacteria and recently he had steno malto (Stenotrophomonas maltophilial which falls under pseudomonas, also a gram negative bacilli). On the chart below it says that Cipro can take care of it. But as stated, he's already been on it twice recently. Worst case he will need an antibiotic IV and need to be in the hospital. But first they will probably just have him take Cipro again. Unfortunately having a trach it's just really easy to get infections because they breed in a wet environment.
