Day 665 - Emergency Trach Change ⚠️ & James Meets Up with Friends at the Park 🏞️
Wednesday, October 26th, 2022
So last night we had a scare. James needed a large increase in oxygen. We went from 0 to .75. Mom posted the following (pretty late) in the Mom of Trach Babies FB group to get some answers...
"*Tobramycin (Tobi)* - So we just started Tobi last Thursday (28 days on and 28 days off) as a preventative antibiotic due to him getting trach infections every month and needing Cipro. Ever since we started we have been getting a ton of secretions, like we are suctioning more than ever. Normally James is off of oxygen during the day and on most nights. Well tonight he dipped into the '80s and I did all the things... changed the pulse ox probe 3x, switched to a different pulse oximeter (same read on different device), suctioned but no secretions, vented his G-tube, emergency trach change (no mucus plug). So I ended up increasing his oxygen from nothing to .75 liters. I know it may not seem like a lot but it just doesn't make any sense. Has anyone else had this side effects from Tobi? Honestly it seems like the cons outweigh the pros with this antibiotic. Also he slept through all of this and doesn't have any lung retractions, so we decided not to go to the ER."
Unfortunately, we didn't get any responses.

Today Mom drove James solo to the park to meet up with friends. During this time we also did his morning trial off the ventilator, so that the majority of the time at the park he wouldn't be tethered. This would also be easier to load him in the car. Now let's park was only 6 minutes away from the house but during that time Mom needed a pull over and suction James because the sound of the secretions we're pretty loud. The other trach baby had quite a long drive coming from across the bridge and came along with her Mom and nurse. We were going to be switching respiratory supplies. We both prefer the trach ties that the other's DME supplies. A lot of times your DME is not based on location as much as it is on what insurance you have.
This was James' first experience on a swing and at first we thought he would cry but he loved it the entire time! [Video below] We thought the weather would be decent but with the humidity James and Mom were both sweating pretty bad. 🥵 We did a full lap around Al Lopez park. 🌳
Afterwards Mom took James for a little drive to get some lunch and Starbucks. 🚗 ☕ It was like a normal Mommy & Me outing. James was doing so good in the back and Mom had the video camera on him. He wasn't very fond of waiting in line at a drive thru or in traffic. James wanted the car to move! 😂
When we arrived home Mom uploaded the car with all of the equipment and James. He then took a long nap. 😴 Later in the evening when we did his 2nd trial off the vent we took him upstairs to see his nursery again hoping he wouldn't be overwhelmed this time! He wasn't! He loved looking around at the wall decals of woodland animals. 🐿️🌛🐇☀️ 🐀 Then James watched some TV with us upstairs. 📺 A rare occasion!
