Day 667 - Halloween Costume & Speech Therapy Update

Friday, October 28th, 2022 So yesterday we had Speech therapy, whoops! These days are all running together. It was the first time James had used a Passy Muir Valve off the vent. Previously James was using an in-line PMV. Again, these are allowing James to inhale through his trach but only allowing him to exhale through his nose and mouth. Below are the consult notes that were sent by the Speech Therapist afterwards. This is a brief summary and definitely doesn't encompass all of what James can do, just mainly a required document for the program. Successes: James continues to make vocalizations, He will make signs for more and toy. James enjoys eating and will stick his tongue out to taste food. He is able to use an open cup when assisted. When he was tired he waved good-bye. He will independently transfer supine to sitting and rotate in sitting to get a toy. Challenges: James continues to reach instead of point with 1 finger. Day 666 - Attempting to Scoot While Sitting & James Favorite Waitress 🍣

Thursday, October 27th, 2022 Today James had physical therapy. We let him scoot all over the floor. He was on a mission. We are trying to get him to scoot sitting versus lying down. He understands, "Hands" means putting your hands down on the floor in front of you and "Scoot" he knows means he is supposed to move forward. We haven't grasped the the part about actually moving forward but he attempts it by using those muscles and still stay in place. For family dinner night we took James to our favorite sushi 🍣 restaurant since this was the last week she could eat it. 😉 James saw his favorite waitress who always take the time to come over and see him. He just lights up! Amazing, because we both don't speak each other's languages but voice inflection and a smile is all you need. 🗣️ P.S. Day 665 - Emergency Trach Change ⚠️ & James Meets Up with Friends at the Park 🏞️

Wednesday, October 26th, 2022 So last night we had a scare. James needed a large increase in oxygen. We went from 0 to .75. Mom posted the following (pretty late) in the Mom of Trach Babies FB group to get some answers... "*Tobramycin (Tobi)* -  So we just started Tobi last Thursday (28 days on and 28 days off) as a preventative antibiotic due to him getting trach infections every month and needing Cipro. Ever since we started we have been getting a ton of secretions, like we are suctioning more than ever. Normally James is off of oxygen during the day and on most nights. Well tonight he dipped into the '80s and I did all the things... changed the pulse ox probe 3x, switched to a different pulse oximeter (same read on different device), suctioned but no secretions, vented his G-tube, emergency trach change (no mucus plug). So I ended up increasing his oxygen from nothing to .75 liters. I know it may not seem like a lot but it just doesn't make any sense. Has anyone else ha...
